In the client under the tab "Options" you must have "Start at Bootup" selected. This will create a service named "Prime95 Service" that starts up the client.
The client it self has a startup delay of 90 seconds by default by startup of Windows.

If you are using Vista, you will only see "Start at Logon". If you want it to start as a tradition service on Vista, there is a special service-only version of Prime95 that will work:
Windows service: ftp://mersenne.org/gimps/winnt259.zip
Windows 64-bit service: ftp://mersenne.org/gimps/win64nt259.zip

Using the above links, I've bundled the service version with a default SB prime.txt file and added them to the download page:

Can someone run them and let me know that they work as is?
